The Anatomy of the Follicular Unit and the Concept of a Living Organ

It is fundamental for the patient to understand that we do not extract individual "strands of hair." What we transplant are Follicular Units (FUs)—complex biological structures containing sebaceous glands, nerves, a small arrector pili muscle, and, most importantly, the stem cells within the follicular bulge.

Technically, every single extraction is the transplant of a micro-organ. If the surgical instrument strikes the bulb or fragments the structure during harvesting (a process known as transection), that follicle will struggle to survive. Therefore, our approach focuses on the absolute minimization of tissue trauma.

Extraction Technology: The Balance Between Punch and Pressure

Many clinics opt for high-speed automated systems to accelerate the process. In our practice, we prioritize manual and/or low-rotation motorized FUE (Follicular Unit Extraction) techniques, tailoring the punch diameter to the unique scalp characteristics of each patient.

• Diameter Precision: We typically use instruments with tips between 0.7mm and 0.9mm. A punch that is too wide leaves visible scarring (a "white dot" appearance), while one that is too narrow increases the risk of cutting the follicle from the inside.

• Depth Control: The incision depth is adjusted millimeter by millimeter. We need to release the dermal attachments without touching the base of the root, where the hair’s vitality resides.

Managing the Donor Area: A Finite Raw Material

One of the most common mistakes in the industry is over-harvesting. A patient’s donor area is a limited resource—a "savings account" that cannot be replenished. If we extract too many units from a single zone, the scalp will take on a thinned-out appearance, compromising the possibility of future interventions should baldness continue to progress.

The time a follicle spends outside the body is its period of greatest vulnerability. As soon as it is extracted, each unit is immediately placed into a hypothermic preservation solution enriched with nutrients that mimic the human physiological environment. This care prevents oxidative stress and guarantees that, at the moment of implantation, the cells are ready to resume their growth cycle immediately.​

FUE vs. DHI: When Extraction Dictates Implantation

While extraction forms the foundation, the placement method defines the finish. Here at the clinic, the transition from extraction to implantation can be performed via DHI (Direct Hair Implantation). In this method, we utilize Implanteners—high-precision tools that allow us to control the angle, direction, and depth of each strand without the need to create prior incisions (channel opening).

This technique is particularly advantageous for patients looking to increase density in specific areas without shaving their existing hair, allowing for a seamless integration with natural strands. Additionally, the implantation process becomes faster for the patient, and the healing period is accelerated since the follicular unit is inserted into the implantation zone in a single movement.​
